Events Request Form

This form is required for any Heller meeting or event, including in-person, dual-mode and virtual.  By submitting this form, we can work to minimize conflicts between different online events at Heller, and your event can be added to the online Heller Events Calendar. Before finalizing the date and time of your event, you must submit this request form, and your request must be approved.

If your request is for an academic program committee meeting, an academic program town hall, TA session, study group, or if you wish to reserve room G51 or 147: Please fill out the Academic Classroom Request Form.
