National Center on Advancing Person- Centered Practices and Systems (NCAPPS) is a new initiative from the Administration for Community Living and the Centers for Medicare & Medicaid Services that helps States, Tribes, and Territories implement person-centered thinking, planning, and practice in line with U.S. Department of Health and Human Services policy. This page features the NCAPPS kickoff webinar featuring Beth Mount on person centered vs. system centered approaches.

National Core Indicators is an effort by State Medicaid, aging, and disability agencies to measure and track their performance. These are standard measures used across states to assess the outcomes of services provided to individuals and families, addressing key areas of concerns.

The homepage for the RAISE Family Caregiving Advisory Council. The council has developed a national strategy for family caregivers revolving around promoting person-centered care, assessment and service planning, information and education for caregivers and supports, respite options, and financial security and workplace issues.

The homepage for the Rehabilitation Research and Training Center on HCBS Outcome Measurement. The RTC/OM seeks to validate and refine a national framework of HCBS outcomes for people with disabilities and implement a framework to assess the impact of HCBS on the lives of people with disabilities.
