National Center on Advancing Person-Centered Practices and Systems

National Center on Advancing Person- Centered Practices and Systems (NCAPPS) is a new initiative from the Administration for Community Living and the Centers for Medicare & Medicaid Services that helps States, Tribes, and Territories implement person-centered thinking, planning, and practice in line with U.S. Department of Health and Human Services policy. National Core Indicators- Aging and Disability

National Core Indicators is an effort by State Medicaid, aging, and disability agencies to measure and track their performance. These are standard measures used across states to assess the outcomes of services provided to individuals and families, addressing key areas of concerns.
